If age 18-39, and no “problems” to review, 40 Min Annual Visit (New Patient) $350

Established Patients:

30 Minute Problem or Follow Up Visit $250

20 Minute Problem or Follow Up Visit $160

15 Minute Problem or Follow Up Visit $125

Subsequent Year Annual Exam Only 40 min $400
